Ulthera (Ultherapy by Merz Aesthetics) is a premium HIFU device with unique visualization: the physician sees exactly which depth they're treating. It's the only HIFU with FDA clearance for face, neck and brow lifting through the SMAS layer at 4.5 mm. Single procedure, expensive (often $2000–4000 in the US), effect builds over 3–6 months. Accent Prime (Alma Lasers) is a multifunctional RF + ultrasound platform working in the dermis (1–4 mm), delivering quality rejuvenation, oval lifting and wrinkle correction without reaching the SMAS. At the consultation the physician assesses skin condition, zones, goal and contraindications. If the goal is deep SMAS lifting, we recommend a HIFU device or thread lifting — we don't offer those. If the goal is rejuvenation, oval lifting, nasolabial folds, jowls or skin quality — we pick the optimal Accent Prime protocol: UltraFace for needle-free RF, Microplasma for fractional work, or a combination.
Ulthera uses MFU-V (Micro-Focused Ultrasound with Visualization): focused ultrasound pinpoint-heats tissue to 60–70°C at 1.5, 3.0 and 4.5 mm depths. Real-time visualization lets the physician see the layers. Accent Prime is a different class: radiofrequency (UniPolar RF) uniformly heats the dermis to 40–43°C across a broad area, while cold ultrasound (Sharewave) additionally works on subcutaneous fat. Microplasma is RF via microneedles for skin quality.

Ulthera is for pronounced ptosis and SMAS work in patients 45+ who are willing to pay for a premium result. Accent Prime is for general rejuvenation, skin quality, oval correction and fine lines in patients 30+ who want a gentle cumulative effect with a comfortable procedure.
Not an exact equivalent — these are different methods. But for most face rejuvenation goals (oval, nasolabial, jowls, skin tone) Accent Prime delivers a visually comparable outcome with a course approach. The key difference: Ulthera works on the SMAS, Accent works in the dermis.
